How to fill out form SSA-3368
Form SSA-3368 is the basic form used for adults who are claiming disability benefits. You can fill the form out yourself or have a representative such as a Social Security Disability lawyer help you with it, but you should not have your doctor fill out any of it. The SSA has other forms which they will ask your doctor to fill out on your behalf. You should include copies of all of your pertinent medical records when you turn this form in.

Much of Form SSA-3368 is simple and straightforward. You should always answer all questions as completely as you can. Don’t leave anything blank. If any section does not apply to you, write N/A in it.

While the form isn’t complicated, there are some things you should know about filling it out, especially if you do it yourself

Rolex Bubbleback Rose Gold Rolex Oyster case is approximately 32.5mm (excluding crown). Reference 3372. Circa 1945.

A waterproof watch with a screw down crown was a novelty in the early 20th century, and customers did not understand the importance of keeping the crown secured before getting the watch wet. Since the crown would have to be unscrewed daily to wind the watch, customers had ample chance to forget to screw it back in, and many did, then attempted to return their waterproof watches to jewelers as being “defective”.

Rolex had built a watch with an accurate movement and waterproof case, but they had to come up with a solution to their problem with user error. Enter the automatic winding watch, which only required the crown to be opened for occasional resetting of the time.

Instead of reinventing the wheel, Rolex attached an auto-winding mechanism to an existing manual movement, and the thickness that this arrangement added necessitated a bulging case back, the distinct feature that earned the Bubbleback moniker. Many of Rolex's competitors used automatic movements built from the ground up, yet this proved to be a poor approach for two important reasons. Firstly, the Rolex movements could be wound manually from the crown unlike the competing models - a feature that was popular with customers who didn’t yet trust the new invention. Secondly, the manually-wound base was easily recognizable by watchmakers who were often hesitant to work on the new-fangled automatic movements.

Roy E. Downham Hall
Roy Downham Hall opened in 1994 and is recently remodeled, offering fully-furnished rooms with a private bathroom and shower, vanity area and sink, in a room shared by two students. It is the largest residence hall on campus, with over 190 students. The building, located near the tennis courts, Kress Center, and Mauthe Center, has added features including a lobby with a large-screen television, study and lounge areas, a large kitchen, and a multi-purpose meeting room with a large-screen television on the third floor.

Roy E. Downham was born in Antigo, Wisconsin, and in 1946 graduated from Clintonville High School. A Navy veteran, he attended the University of Wisconsin on the G.I. Bill. He received his bachelor of science degree in mechanical engineering with honors in June 1952. His long and prolific professional career started with The Trane Company of La Crosse shortly after graduation and included engineering positions with Ansul Chemical Company, Marinette, and Overly's Inc., of Neenah. While with Ansul during the late 1950's, Mr. Downham taught engineering drawing and descriptive geometry courses at the UW center in Marinette. In 1969 he co-founded TEC Systems, Inc., in De Pere, and served as its president until retirement in 1982. In 1986, Mr. Downham founded Advance Systems, Inc., (ASI) in Green Bay, for the design and manufacture of high-velocity air flotation dryers and related web-handling equipment for the graphic arts, converting, and paper industries.

Mr. Downham is credited as the inventor of the first disposable dry chemical fire extinguisher. He was instrumental in the development of high velocity hot air dryers for graphic arts, converting, and paper industries, and was the designer of the first compact flotation air dryer used in graphic arts for the drying of web offset printed webs. Mr. Downham holds numerous patents relating to air dryers and flotation devices.

Along with Robert Schaefer, James Temp, Donald Long and the late John Robishaw, Mr. Downham founded the non-profit board, University Village Housing, Inc. to develop residence hall housing for students at the University of Wisconsin-Green Bay. Mr. Downham has shared his substantial professional engineering knowledge during both planning and construction phases of the University's ten original halls, and the hall that bears his name. For well over a decade, he has given freely of his expertise and worked closely with architects and contractors to assure that the highest quality housing is provided for students of UW-Green Bay.

After being retired, the locomotive was sold to the Edaville Railroad in September 1961, and then was later moved to Bellows Falls, Vermont and became part of the Steamtown, U.S.A. collection in 1969.
It has been cannibalized for parts for Steamtown's operating Canadian National 2-8-2, the 3254.
Even the pilot wheels and trailing truck were removed, making it an oversized 0-8-0.
The locomotive sits on display south of the Steamtown shops.

Priest’s dish, iBuburaunibete
Carved from a solid piece of vesi wood and standing on intricately carved pedestal bases, these yaqona dishes were used by priests.
While the form of the upper part of the dish remains relatively constant, the variety of shapes of the pedestal base testifies to the creativity of Fijian wood-carvers.
A few duck-shaped yaqona dishes are known.
This one was probably used for scented oil rather than yaqona.

If you're interested in a job as a food service specialist in the Marines, rest assured you'll be doing more than peeling potatoes.
This is an important job in the armed services: Marines (and soldiers, sailors, and airmen) won't get very far without being properly fed.

Food service specialists, which are military occupational specialty (MOS) 3381, are involved in every aspect of food preparation from procurement to storage and distribution, in both garrison and field environments.
Their duties extend to funding, requisitioning, purchasing, receiving, accounting, and storing food supplies, along with of course cooking, baking, and serving.

Marine food service specialists could potentially be stationed anywhere the Marine Corps has a need.
All Marines need to eat, so wherever there are Marines deployed, a food service specialist could possibly be sent there as well

Vintage bus rides
Vintage double-decker bus rides depart regularly from the main Museum entrance for a 30-minute round trip to the Queen Victoria Building (QVB) and return. Visitors can join the buses at either the QVB or from the Museum.

In 1929 the White Motor Corporation of Ohio, USA, introduced a double drop frame coach chassis, with a 6-cylinder petrol engine and 250 inch wheelbase. The local Australian dealer, Dalgety & Co, had to have NSW road regulations relaxed before the trial 54A could be introduced – at that time, it was much larger and more powerful than any previous bus in service in Australia. It was fitted with Westinghouse air brakes, one of the earliest applications on a road vehicle in Australia. The brake system was derived from railway technology.
The prototype chassis arrived in Australia on 28 January 1930 and was sent to Walsh Island Dockyard in Newcastle, where they produced the first all-metal bus body constructed in Australia. On April 15th 1930, the bus entered service for Mr Michael Fitzgibbon, who operated Route 42 between Campsie and the City under the name ‘Deluxe Bus Service’.
Three additional White 54A chassis arrived in Australia on March 22nd 1930, also for Fitzgibbon’s Deluxe Bus Service. These were bodied identically to the prototype at Walsh Island.
The Whites served on Route 42 until October 31 1931, when legislation preventing the operation of bus services in competition to NSW government tram and rail services became effective. The vehicles were placed into storage for five years until March 1st 1937, when the Department of Raod Transport & Tramways purchased all four of the White 54A buses from Fitzgibbon.
The bus was registered m/o 1500 by the DRTT, and operated until February 1939 when it was sold to Rover Motors, Cessnock.
It later passed to Quodling Brothers, Queanbeyan who recognised the significance of the bus and donated it to the Museum’s predecessor organisation in the mid 1960s.
The vehicle is in the middle of a thorough cosmetic restoration being carried out by our volunteers.

Engine: White six-cylinder overhead valve petrol engine, bore 4.375”, stroke 5.570”, 519 cubic inch displacement with dual spark ignition developing 130 brake horsepower at 1200 rpm.
Transmission: Four speed sliding type ‘crash’ gearbox to a crown wheel and pinion differential.
