I think he may have lost his head as a result of the club wanting him to look more friendly for children - , because I understand he visits hospitals, schools etc. No idea about the first question though.

Am I the only one left guessing here ? I think he has been the mascot for about 15 years. Was his previous head too scary for children ? I know there was some dispute between him and the club about 5 years ago. Am I getting anywhere close ?
Think I will have to say as you are getting close. 1995 he first appeared. The original Harry was more scary and the club wanted him to be more friendly. The person inside the outfit was asked to return it but was not happy, so returned the costume but without the head.
