All his mistakes are because he feels uncomfortable with his positioning and it's usually when he's come over to the right side and is concerned about the massive space he's leaving over to his left. Normally he'd not have to worry about that as there would be the left side centre half (Cooper or Struijk) to cover. So yes his mind is elsewhere as he's out of his natural position and it's exactly the same when in possession as well. Not sure if you're aware but he's not very good with his left foot and so he can't just turn inside and play the square pass to Cooper etc as he would usually, instead he's limited to looking solely to his right which severely limits his options.
Just play him in the position he's played his whole career and he'll be fine.

If he doesn't understand that when you are under pressure in possession of the ball it's time to get rid then it's curtains.
If he doesn't understand that he has to sweep behind his partner when the big centre forward has dominated all game in the air then its curtains.
If he doesn't understand when his manager has talked all week about giving cheap free kicks away that it's a bad idea to give cheap free kicks away then it's curtains.
If he continuously gives possession away or charged down as last man because he can't use his weaker foot its curtains.
Sorry mate, he's making errors that I got taught not to in the u12's. His confidence is shot. Get him out of there.
