It's not that hard (with a little research - or you know your music history) and it is a darkly humourous tale....

I'm glad you allowed research....

1. Dead Again
2. He had previously been thought to have died when his band's website published an image of a tombstone bearing his name and the dates 1962-2005
3. He actually did die later from heart failure probably connected to his heavy drug use.
 
Had to allow research BB, far too vague otherwise!

The irony is , of course, that Dead Again was their last album. Tragic waste of a darkly funny musical talent: they have a recording of Black Sabbath's "Paranoid" which they managed to stretch out for nearly 9 minutes - the original zips by at a pacey 2 1/2 mins.

He is the man in my 'signature'.

It wasn't until 1989 that the world realised that I am poisonous - thanks to this man who accidentally found out. I'm now known as the only one of my kind to be poisonous.
