You seem to have read this with an agenda. It actually says lack of de-escalation, lack of patience, etc. Escalating is not failing to deescalate. I know your love of semantics, so I can't wait to see how you justify this misread of the article.
The memo also states that force used by officers on members of the public could probably have been avoided in some instances.
