It would be funny if Manu or Arsenal scrape 4th ahead of Citeh, but miss out on CL should Citeh win it.

All those above minus malaga plus juve roma dortmund basel galatasray etc did win
Maybe the case. But it's rare as you said.
It would be a boring sport if the favourites won every game. Things like Chelsea finishing in the top 4 might happen.
I think the safe bets are usually PSG, Real, Barca, Munich. Other than that it's hard to call. But those 4 offer pointless odd's so it's not worth it just in case one lets you down. Barca for example on recent form (minus last nights Suarez masterclass).
