Danny Murphy (I know he's not the font of all knowledge) said last week in an interview that when players hand in a transfer request they waive any right to bonuses or extras on the contract.

Whilst I'm sure he's handsomely rewarded anyway, I'm sure he's accrued a fair few added extras, and for him to turn his back on that would imply he's had enough of the way things are happening up there.

That's correct; barring any kind of further agreement between player and club, handing in a transfer request forfeits loyalty bonuses and the like (I do so enjoy the idea of paying out loyalty bonuses while the player is being transferred...).
 
On top of that I believe signing on fees are paid over the length of a contract. Handing in a transfer request means the player misses out on a portion of that fee as well.
 
Perhaps, but a new 2-3 year contract at another PL club would ease that sacrifice.
Looking at a quick potted history, he's been around the lower echelons a bit and dare I say Is in the Autumn of his career.... I doubt he'd command a massive fee wherever he went.... I would've thought negotiation with Stoke would be the better way as it would preserve his extras.

Madrid come out worse than Man Utd here. United at least keep their top class keeper for half a season. They may even keep him all season. Assuming he's prepared to play for them.

Real could decide to get De Gea to sign a pre-contract in January and sign him for free next summer.

How have they? Casilla is a good enough keeper. They will now get De Gea for free.
