Spoken English can be quite far removed from written English. Say "could´ve" quickly several times, and the sound is almost like an "f"

I agree, the derivation of “could of” is ironic, as it seems to have come about by people trying to correct the mispronunciation of “could’a”, which is a common idiom. Using it in written speech makes no sense, however.It depends and needs more words around it. The "f" sound is very weak/and often omitted in spoken speech. There will be variations depending on where it is spoken and the "kinda peepil" speaking.
