Leicester definitely and Stoke? Not sure if he played for them that long though

He started in Leicester (he's from there) and went to Stoke, Forest but finished at Derby (in terms of the clubs he mainly played for). The 4th club was presumably between Forest and Derby but I've no idea who it was.

Which footballer represented 4 different clubs at youth level (Leeds, Excelsior, Everton and Man C), went on to play for Man C 37 times, Leicester 7 times, represented England at U16, U19, U21 before quitting the game at the age of 24, eventually becoming an estate agent.
Clue: He was high quality, we were linked regularly and was touted as the next big thing for England.
Leicester, Stoke, Forest, Southampton, Derby. Played over 100 games for all of them. I can't think of anyone doing anything similar.
