There were another two health professionals there who all concurred. How that splits from the East Riding is academic as three for Hull is simply shocking. Put all the information needed for the form in an email, and explain your situation. I reckon they'll do all they can to help. Failing that, PM me that info, I'll write a form out and take it in for you or scan it and send it back for you to email or, daft as it sounds, fill it in and take a photo, or a screen dump and paste it to a word document.

You either have to become a member, or find a member, of club wembley. You can't even look on the club wembley website without being a member.
There was no one there - I collected my tickets and went for an extra ticket for my daughter. The guy didn't even check that I had bought 2 tickets for previous games this season.
But there's the rub; as I said, I won't be kicking myself! If we lose in the semi-final to a Division 3 team I'll be mightily relieved that I DIDN'T go! I mean, can you imagine anything much more disappointing?
Wembley Ticket Update: We are pleased to confirm that all supporters who have applied for tickets for our FA Cup Semi-Final against Sheffield United using our general sale 1st phase application form are guaranteed tickets. These tickets will be ready for collection from the Tiger Leisure Superstore at the KC Stadium on Monday. Alternatively, if you opted for your tickets to be delivered, these will be sent by special delivery, also on Monday. Any Season Card holders planning to collect their Wembley tickets ahead of Saturday’s home game should note that these will be available from the KC Stadium Ticket Office. The Ticket Office will be open from 9am and we’re advising supporters to collect their tickets as early as possible to avoid queues forming close to kick-off time, with the advice being to collect before 1.30pm.
