It just comes from sitting down in the summer and deciding 'what do we need this season?' instead of 'how are we going to build this team for the next 3-5 years?' A manager doesn't have 3 - 5 years and is under pressure for immediate success, so it can only come from the top.
At the danger of reopening old wounds I would say Mick was given ample time,but his attitude very much mirrored that you attribute to the owner,i.e. short-termism with no thought to a more cohesive long term plan.
I don't think McCarthy expected to be given the best part of six years, or I'm sure he would have made some different choices.
