Can be bad luck as much as anything else, look at Spurs - lost their 2 best players in the same game, and they've fallen off the cliff straight afterwards. We're fortunate in that we can still put out a winning team despite this, and players are coming back now.
I think it's likely that extra minutes in the games could be adding to the injury count. Player have been training for 90 +3 mins and now you're seeing loads more game time Think it was probably that the team were working hard together and he didn't want to interrupt what was going on.

There's no helping these people they're infatuated. I remember suggesting Howe should rest Pope before he got sent off via Liverpool and after the red card I was told it was hindsight that was required to avoid losing him for the final... it wasn't though. Essentially there's nothing Howe does that should be questioned because in their minds he is their new messiah. If he changed something he would have changed what happened next. Unfortunately his plan didn't work but as we know it should have.
Personally I think most top managers would've made changes and tried to break up the momentum of the added on time. Change the keeper, change striker for a defender just break up the play. He didn't and unfortunately they won a penalty.
