Any action taken should be taken against them - they are supposed to be adults after all - club management teams aren't their nannies... I can't even remember who started it now...
I doubt it. The fines go up for clubs with a history of this - subsequent incidents attract bigger fines - the players in those subsequent incidents may not have been involved in previous ones - may not even have been at the club when they happened.
