In my opinion, the goalkeeper does need to be particularly vocal as they'll have the best view of what's going on. Hopefully they'll both learn from this error and it'll stop similar things happening from now on.

Yes, just know from my experiences when I played in goal- I was too quite and perhaps too respectful of my centre back and captain, but there are situations when he needed me as he didn't have eyes in the back of his head- He would expect a shout which often didn't come and then I would get it in the neck- Flint seemed to suggest that O'Donnell should talk, he seems ok to me but I hope this was a one off and not a potential problem
