If you look at it, the ref definitely didn't see it as he was half way down the pitch running the other direction, and Cook had his back to the lino and is ten inches taller than the Swans player, so the lino couldn't have seen anything either. It was based completely on the much large player throwing himself to the floor.
A complete guess, may as well have played rock/paper/scissors for it.
