The world is awash with rich, powerful people, who don't follow the rules laid down for the rest of us. They don't expect to have to do that.
They can go where they want, do what they want, whenever they want.
Networking is key to their activities, and we don't really have a clue as to how organised that is.
Epstein was part of the network, and he had powerful friends, which is how he got away with it.
However, once he became ensnared in a very public scandal, and other big hitters were being threatened with exposure, he had to go, hence a visit to his cell by persons unknown.
Excess is what they crave, and excess is what they get. Don't threaten the excess, or else.

Remember how easy it was for the British establishment to arrange for that crash in that Paris tunnel?
The victim had become a major threat to the British establishment. She had to go.
