It's all been a pointless exercise. The sensible thing to do would have been to investigate the advantages and disadvantages of the name change; if there was good reason to change the name then publicise the reasons; then have a vote of season cardholders; then if they get broad support apply to the FA to change the name. The Allam's seem to have a different mindset.
No, it must be totally independent. Try Russians they are good at getting the required result. I'm sure they could announce the results in time for the submission to the FA and even before they have to send out ballot papers.
Its obviously going to be a vote of confidence but you wont be able to vote against the name change without voting against the allams. So while the vote doesnt sort the main issue out he will use the results to appeal to the fa??

Considering the question is bound to biased in some way, there's hardly any chance the FA will listen to it. If it was a simple yes or no question, they might take notice. But when your question is bound to have some form of blackmail involved there's no chance.

The Allams will win the pass holders vote by a margin of 58/42, which will allow them to say they have the fans onside, which will also save face, which will allow them to continue as owners of the club even though the FA will reject the name change.
