Yeah, there is a template at the bottom, comment your answers. I have set it to email me when a comment is made so I can note down name and marks, before selecting not to publish your comment!
 
To comment, do I select the Name/Url option and enter my name? I think it's sent but not sure.

With the trial I selected anonymous and wrote name at the top

UPDATED: Just checked it again, yeah just comment, put your Name in, don't worry with URL.

It sends me an email asking for me to read comment then trash or publish it.
