For some reason I have in my head Puncheon got relegated a few times - I would definitely guess shearer for pens - although French Eric seemed to score a lot of pens
I was pretty sure that (5) was James Milner. However, as of last weekend he has only scored in 45 games in the PL and never been on the losing side. He could be equaling or beating Vassell soon.
Penalties has to be Le Tiss. His record from the spot was outstanding. I think Rickie Lambert had a similarly impressive penalty scoring record at Southampton? He scored all 30 odd penalties without missing one - he missed his first penalty as a Liverpool player. Le Tissier was one of my favourite ever non City players, I was privileged enough to go and watch him play in 'that' game at Ewood Park versus Sutton and Shearer in the mid 90s. A wonderful footballer who made the game look really easy, I also admired the fact he was a one club man, too many players jump at the chance of a few hundred extra quid a month.
Back then it probably only was a couple of hundred quid a month, nowadays he'd be on a couple of hundred grand extra a month.

The fact there's an assist as well a goal makes me think it's more likely to be strikers and midfielders that put in own goals at set pieces than defenders who scored. Someone like Chris Sutton. Although if it is defenders Ian Harte and Christian Ziege would be my guesses. Freekick/corner taking full backs
