How much is his value in marketing though? Despite apparently being the 3rd best player in the world, globally he wasn't even in the top 10 of Premier League player shirts sold.

I imagine he'd be quite marketable for Real given their stature in world football but does he really add much to Spurs currently?

He sells no shirts because he plays for spurs. As soon as he moves to a big club he will sell tops. Hazard and even Torres sell far more shirts
 
Looks like Spurs have secured the services of French central defender/midfielder Étienne Capoue for around EURO 8m from Toulouse.

If true, he represents a big step-up from Steven Caulker, who Spurs recently sold to Cardiff City for GBP 9m.

He's primarily a midfielder, so not really a like-for-like replacement for Caulker.
