It means that there's a result (or set of results) that means regardless of what result you achieve, it's rendered impossible for you to succeed.
But, there's no such result.
We simply need to win (and assuming that they also win), better Wrexham's result by one goal.

In our hands means that the outcome is under our direct control. Well it isn't under our direct control because both Wrexham and Derby can scupper our chances on the day. If our game was starting 2 hours after the Wrexham and Derby games then the outcome would be directly under our control because we would know exactly what we had to do.
However it isn't out of our hands because we still have to contribute as best we can to the final reckoning and we have no control over what Derby and Wrexham will contribute.
