It's generally caused by drivers not thinking further ahead than their bonnet. You are in the outside lane, with a bigger vehicle in front of you, the bigger vehicle changes lane, you accelerate to over-take, not knowing the vehicle in front of you changed lane to avoid a broken down vehicle, and you'll have zero reaction time to do the same. It's exactly how pile ups happen, someone suddenly changing lane at the last second, leading you into the back of the last vehicle in a queue. It's going to be down to your driving experience to avoid those situations, and it's bad luck when you don't, but sadly fingers are still going to point at you for not paying attention or driving too fast. There's an example on youtube somewhere, where a car changed all three lanes at speed, but he allowed himself to be blinded by a large lorry as he overtook it at speed, and the final lane became fatal.Interesting one ...

For me, both the accused AND the Highways Agency have to carry some blame ... but ultimately, if the van had its hazard lights on and he plows into doing 77 mph, he has to take responsibility...
The Micra driver was at least partly culpable probably. Sounds like she and her dad were clueless and it’s sadly cost the bloke his life, severely injured the van driver and ruined his life. Will probably be driving the wrong way round the Morrisons car park not seeing zebra crossings by next week.
So it was. Still, Micra, middle-aged Asian woman driving, nervous on the motorway. Bloke was probably eating a Greggs and getting wound up by Talksport at the time but he’s done that 1000 times and been fine.Turns out it was 2 women in the micra (mother and daughter)... and the van driver wasn't wearing a seat belt...
Just had a look at the photo from the embedded link in that article, where the Micra was broken down in the outside lane, didn't stand a chance tbph. And there was no where for them to go for safety. Concrete barrier central, so no centre steel barrier to climb between and no hard shoulder, everything that was the concern with smart motorways, but the past government wanted to cut costs...

I remember them doing all this to the M25 ready for when we had the Olympics, I'm sure parts of it lost it's hard shoulder. Although in fairness I don't think a hard shoulder would have saved that couple. That's just bad luck and the odds of survival are against you. If the traffic had been heavier it would have had a natural slowing and building of traffic to protect them, but you can see from the picture the traffic is light and sadly that was their undoing, because speeds will be higher and then it's a dice with death.Anything but 'smart'...
