At one time, weren't you only allowed to substitute an injured player who couldn't continue, and not because you just wanted to?
I think the first season a player had to be injured to be substituted, thereafter the sub could come on for any reason.
Clearly no one knows so.....
First sub in 1965 for injury only - Keith Peacock of Charlton who replaced their injured keeper.
1967-68 it changed to allow tactical subs.
Can't find out City's first sub. Birdie signed for us in 1966 so may have been first tactical sub but we are likely to have had an injury sub before then.

Ok then here goes, our first sub was David Summerhayes on the opening day of the 65/66 season, same day as Peacock claimed to be the first ever League sub. Summerhayes came on for the injured Colin Baker in a 1-0 home win against Bury. And our scorer was the great John Charles.
Summerhayes was a Llanishen lad and his younger brother, Robert, also played for City.
