I think Q6 is broken, unless you meant to make it like that. I can only put 1 tick in each column (i.e. if I put a 5 for one answer, I can't use 5 any more) which means I can't put a tick in every box.
question 6, it says you can leave blank if not important but wont allow you to go onto the next page if you have only answered against 1 or 2 points
