The one to Dubravka made me think of a way to potentially improve the game: Injuries to a keeper should result in an enforced, temporary sub. I've said for ages that outfield players requiring treatment shouldn't stop the clock. Carry on playing and allow the physios on. If a keeper needs treatment, then he should have to come off to get it and his backup should come on while it happens. Problems? Teams might start using it as a timewasting tactic. I'm sure there's a way around that, though.
What, carry on playing while the physios are treating a bloke in the middle of the pitch who’s knee has been snapped? Dodge the stretcher bearers? Not sure you’ve thought this one through.

Give an exception in cases where there's a serious, obvious injury and not just some twat rolling around to run the clock down.
Depends on the injury and how it's caused. If it's anything intentional and nasty, then it should be dealt with by the officials anyway.

How does bringing another player on, who’d have to get prepared etc not disrupt the flow? Or are you suggesting that the side who’s player had been kicked etc and needs treatment should just suck it up and play with 10? No mate. The timekeeping is a separate issue.
The timekeeping is the same issue. We're not going to end up with games lasting as long as they'd have to in order to stop timewasting. Refs aren't going to be adding on 15 minutes regularly or something equally silly. Make it pointless or damaging to timewaste, as it should be. I'm sick of seeing keepers running down the clock in the first minute and faked injuries to stop the game.
