It seems to be a DVT - deep vein thrombosis - in his leg. I suffered the same complaint in 1980 and 1982; I had two by-pass ops in my leg, but they may be able to bust his clot/s these days, without an op. It's usually only suffered in 'later life', but can be brought on by life-style...

That may be the case which makes me question the original ongoing medical advice from when he was 15, I would expect daily aspirin after months on warfarin etc, but I am referring to it being an ongoing situation that requires maintenance now going forward, which it most definitely is. A second incident usually means you are in blood thinners for life, so ongoing maintenance.
