It's the recovery process having had a limb immobilised for a period of time. Rebuilding muscle wastage, it can take you a fair bit beyond the period with which the cast was removed, it did in my personal experience anyway. Walked like a cripple for about 6 weeks after - let alone play football!

Yes the rule only stops loaning between two premier league clubs. We can send them on loan anywhere else we like.
