I've had some time on my hands over the last day or so, so looked into this properly. Division Four was established in the 1958/59 season, since then 94 clubs have played in the fourth tier under its various names. Of those 94, 32 clubs have also played in Division One/Premier League since WW2. In that time, there have been 58 players that have played for one club in all four divisions. The first five to achieve this played for Northampton Town in the 1965/66 season, the most recent were Pelly Mpanzu and Luke Berry at Luton Town this season (there are two more current Luton Town players, Dan Potts and James Shea, yet to make their PL debuts who would also qualify). The teams with the most players on the list are Wimbledon (eight) and Swansea City (eleven). As well as the four players who achieved this feat for the Tigers in 2008/09 season, there are four other players on the list with Hull City connections - Terry Dolan (achieved the feat at Huddersfield Town), Keith Edwards (at Sheffield United across two spells), Kevin Gage (at Wimbledon) and Glyn Hodges (also at Wimbledon).
Technically appearances were possible for one club in all four divisions prior to 58/59. Teams in the midlands could be moved between 3rd Div North and 3rd Div South, to even up the divisions after clubs were relegated from 2nd Div.
Coventry were the only club to have played in every division including Div3 North and South, First Division and PL as far as I know..
that's true, although the regional third divisions were equal, so there wasn't a way to ascend or descend three levels within the league.
