I worked at that base for 5 years didn't know that guy though it happened after I had left. I agree though surely people climbing into bins is not common and therefore not a safety issue but more of an idiot issue?
I think people tend to forget there is a risk to everything in life, even more so if you go doing dumb things. There was an article in the Kent rag recently as it was 30 years to the day of one of the biggest fires ever in Maidstone, one that I remember well. In the previous years there had been 5 to 10 years of massive development of a large housing estate and subsequent infastructure of new shops, including a large Tesco's - that was until late one summers day a couple of kids decided to set light to either a bin or skip, the fire subsequently spread and completely burnt down the Tesco stores and near-by retail units. Sadly you can't account for things like this in life, if people go doing dumb things, you risk getting bad outcomes. Much like the number of crush injuries you see around bins and skips, if there's truck reversing to pick something up....move, get out the way and certainly don't go sleeping in them.
One of the places we are contracted too is this huge building, massive, we have to go around and make sure it's all locked up and alarms are on. One of the things we lock up are the bins, we are told to triple check the bins Incase someone is asleep in there. While I will check em, I also would be ****ing livid if someone was crushed and I was asked "why didn't you check?!" I'd say, "why the **** were they in the bin is a better question!" If you're dumb enough to climb in a skip, knowing that they're crushed every couple hours, you're a ****ing div.
