    Virtually boasted about it and got everyone killed with his arrogance.

    I saw on Twitter this morning somebody signed up for a dive last year, but pulled out when they saw three separate references to their potential death on the first page of the waiver

    Okay, first of all, if the contents of a waiver cannot be contained on a single sheet of paper and you are not about to have major invasive surgery, walk away. Here's a short list of waivers that can be done on a single side of paper
    - A visa waiver to enter the US
    - The waiver for skydiving, which has just the one reference to your potential death once you sign
    - The release form for any TV program, which can include anything up to your house being trashed by the Beadle's About crew

    Secondly, as hinted at before, most waivers that make repeated references to your potential death are those issued ahead of surgical procedures, for example when my mother had a pacemaker fitted it was six pages long and had 2-3 references to potential oopsies (which, given this was at mayday, was a very real concern...) - but those 2-3 references were over the entire document, not on the first page
