Both would be effective if implemented. We've seen plenty of rules confe in that had good intentions but implementation doesn't happen. Simply put, the referees face too much flank for following them through so **** out of doing it.
No, that's fair game because the ball is in play. Much like if the keeper as the ball at their feet, the opposition are entitled to attempt to gain possession. You can't gain possession when the keeper has the ball in their hands.
