Off Topic Air Crash Investigation - Your most memorable

    The sequel to the morbid Serial Killer thread, this is about the award winning TV documentary series also known as Mayday. I've always found the range of causes for flights going Pete Tong in every episode fascinating - from using the wrong screws to refit the pilot windscreen, to dodgy altitude meters, to ripped hydraulics leading to aileron failure.

    So anyway, have you ever watched it, and if so which were the most memorable or fascinating?

    This is one of mine... In August 2001, Air Transat Flight 236 from Toronto to Lisbon, runs out of fuel due to to a leak in the fuel line whilst over the Atlantic. The plane was diverted to the Azores and at some point the engines flamed out. Without engine power, the plane lost its primary source of electrical power.

    Something called an "ram air turbine" which is basically a stick with a wind operated propeller drops down from below the plane" This provide essential power for critical sensors and instruments to fly the aircraft. However, the aircraft lost its main hydraulic power, which operates the flaps, alternate brakes, and spoilers.

    The two pilots then proceeded to glide the Airbus A330 across the Atlantic towards the island <yikes> Thermals and natural rate of descent were calculated as they went along. They just about made it to a military airbase iirc where they had to descend unconventionally factoring in the end of the runway which was a drop to the ocean. But they managed it, bursting all 8 tyres. Only two passengers suffered serious injuries and that was during the evacuation of the aircraft!

    Fun fact: The captain had in the early 80's served 16 months of a 10 year prison sentence for drug smuggling from Jamaica into the U.S.

    Fun fact: I've been on board a flight that had to be diverted and make an emergency landing in Oman due to a bomb scare.

    There is that risk. I think more on here may have seen "How It's Made" than seen Air Crash Investigation.

    I have to say terrorism wasn't the norm on that show and never really what interested me about the programme. Moreso simple engineering maintenance fck ups or design faults to begin with.

    Pretty much every episode highlights how plane design or procedures have been changed as a result of events in an episode. Including something as simple as English terms used worldwide in flight communication and with Air Traffic Control. All because of a language miscommunication on Malaga (I think) which resulted in a passenger plane taking off colliding with a plane landing.

    I hate flying, you’ve just made it worse.

    I take Valium and at least four pints before getting on a plane.

    Had a client was a captain of jumbos for BA. he was the man who came up with the idea that when you hit a flock of birds and lose engines that you put the engines into reverse thrust mid flight. It’s apparently now in the rules for pilots. Also if you hit volcano ash, they do the same.

    The volcano ash episode was very good. The hot ash enters the turbines causing damage to the blades but what's even more interesting is they cool and basically weld to them causing the engines to flame out. I can see why reverse thrust would be a good idea. Also, the hot ash hits the pilot windscreen at such a speed that what the pilots see resembles sparks of white light like they're on the Star Ship Enterprise. The result is that windscreen basically gets scratched to buggery with fine particles causing it to lose transparency and the glass turns cloudy. <whistle>

    Same guy had the windscreen blow out of the cockpit.

    He was Irish and loved the fact he scared me to death.

    Serious? The guy that spent most of the flight outside the aircraft being held in by his arm by the co-pilot.

    I can’t corroborate it. Just what he told me. Had no reason to disbelieve him. Was back in 2005 when I met him.

    Remember watching one on the Silk Air flight that crashed in a river on a flight between Jakarta and Singapore in late 90's. Initially they had said it was a deliberate action and the pilot had committed suicide. The Air Crash investigation suggested that it was actually rudder failure. Apparently the rudder manufacturers settled out of court with the families afterwards. Don't think they ever officially exonerated the pilot though.

    This place must have one of the worst records in the world for airline safety, mainly because of the rapid rise in domestic flights and lack of pilots. Horrific stories about them drug testing pilots before they fly and finding them drugged up to their eyeballs to try and stay awake.

    Considering I fly in the order of 100000miles a year I have been very lucky and not been involved in anything serious. Had part of a wing drop off an Avianca flight from London to Bogota via Caracas once but it landed in Caracas without incident and they repaired it on the ground (14hr wait sunbathing on the roof of the terminal). Didn't actually tell us about it until it landed.

    Once just missed a domestic flight from Arequipa to Lima on Aeroperu and had to get the next flight. While we were waiting in terminal the one we should have been on came back for an emergency landing with one of the engines in flames. Landed and they doused it in foam while the passengers all came down the emergency slides. Thought we were going to get bumped to the next flight so they could accommodate them on our flight but I think they all opted for the 11hr bus ride.

    Not worried about flying just getting sick of the jet lag effects worsening as I get older.

    I've done a bit of flying in South America too. I used to go to Caracas and then get a flight out of there to Valledupar. That was on one of those little twin prop planes that are like old busses out there. They are sketchy as ****, flying through the mountains with pilots that look and sound half pissed. The locals used to say prayers and hail mary's on final descent and then break into a round of applause when you landed safely.

    Scariest one I went on was in a thunderstorm, the plane was getting buffeted up and down by big thermal updraughts from the mountains, with big ****ing cracks of thunder and lightening going on all around. I remember preparing myself to die that that day <laugh>

    Think the worst places for air safety though are some of the African countries. Democratic republic of Congo springs to mind as being one of the worst. But yeah loads of Indonesian airlines have terrible safety records. Most of these are banned from flying in the EU

    I was first on scene for a Hawk going down on Anglesey, it came close to hitting a house (it was empty at the time) pilot had popped out and was in a lot of pain. Crazy to see something like that.
