Apart from Malaysia the other possibility is Andorra, where the title of Co-Prince automatically falls to the President of France.
Not technically a monarch though, Andorra is on the ‘almost’ list like quite a few others. Free fact. The kings of the Polish-Lithuanian Commonwealth were elected for two centuries before the Russians, Germans and Austrians tore the country into bits. But in those two centuries it was the true centre of European culture.

This place does always have a constitutional king from the same Royal family, but when one dies or abdicates a committee mainly of elected officials select a new one from the family, it doesn’t always pass father/mother to eldest son/daughter. And for a time in the seventies they didn’t have a king at all, at least not physically in the country.
