It was taken in September 76 according to google, and first used in a calendar in 77. In that pic above the DM says she was 52 in 2011 which makes her 17/18 when it was taken. So, old enough then. Just!
Yeah, they dropped the Mujahideen part. After having funded \and armed them for a couple of decades to fight the Soviet Union, they then decided that they were the enemy after 9/11
Absolutely. Same as has been happening in Syria, arming the 'rebels' to try and overthrow Assad and then deciding that they are actually ISIS.

Not sure the US Govt would have been directly involved, but wouldn't surprise me if there was some US complicity in the attacks at some level. It certainly gave the US the green light to invade Iraq again. And it wouldn't be the first time the US has been accused of having bloody hands in attacks on it's own people. I think it's more likely it was State funded and orchestrated with help from another country, possibly Iran. With maybe some Afghan / Pakistani religious figures involved. There's so many holes in the 'official' story though. And lots of reputable organisations have questioned the explanations. The whole thing is very shady and murky.
