These players hit the ball a long way and think the person in line to be hit won't hear but as soon as someone shouts fore the instinct is to duck if you're in the vicinity of where the ball is going to land. They or their caddy should just shout it, they're covered then.
You cant hear "Fore" from 300 yards, especially over the noise of the crowd. What the Euro players were doing was holding out the club in their hand to visually warn the crowds which seemed more sensible. You should still shout, it ripples down the crowd before the ball reaches the target. People just instinctively duck if there is a 'fore' in the air. She was probably looking at her phone or some other bird type ****.
