I think one thing that this incident has shown up is that the refs cannot hear anything in the ear either big earpiece and mike in.

Madley turned around during the incident so that ear was half facing firmino at the end and when he went over to the 4th offical he had to take it out.

At this point nobody has a clue what went on it very much seems like Holgate has made an accuation and Firmino will probably deny it, if that's the case it should be left alone unless someone heard and understood clearly what was said. It doesn't look like that is the case as nobody other than Holgate reacted to what was said.
