If a player delays his run up, or faints as if to kick and then delays for a second, then it's a free kick and no re-taken pen. But when that happens the ref is meant to book the penalty taker. No other offense should result in anything other than a re-taken penalty.

I'd never even heard about the wrong player kicking the ball changing from a re-taken pen. But can't believe anyone would do that these days.

Just looked at the FIFA rules and it isn't even illegal to feint. It's only illegal if the referee decides it is also unsporting. Apparently if a player misses the pen then you give a free kick for encroachment - but if he scores it is re-taken. Wonder if the ref got confused between those two?
