Possibly because head vs head nobody leads in with a head challenge carelessly ,recklessly and without expecting to connect with the soft ball .
The bigger difference with foot and head is neither player "leave their head in" "follow through needlessly" that can happen with the foot .
I'm not trying to compare deliberate fouls but those that are considered accidental injuries caused.

Rat hater than head challenges I'd rather you compare with goalkeepers leading with raised knees in the6yd box - these are never called fouls

far too early to have an idea
we don't even know what type of fracture or swelling issues etc

for now it's just about recovering from the initial injury

But he can easily get the ball smashed into his face or a foot in the head during a one on one, or a clash of heads on a corner.

 

Terry had a full surgeons outfit in a bag but didn't want to put them on until he was sure Ryan was going to pull through.
